Stamp User ID against checklist task

I have multiple people working through a checklist at one time. I need to trawl through the audit log to see who completed what task. This isn't helpful for me or the team using the checklist, can we record the User ID on the front page of the checklist?

We wanted in the same lines. Instead of logins, we want a secret code before any one can fill in the checklist. As no other person indented to fill the checklist is allowed to fill.
